We are thrilled to announce the launch of our new 12-Month Premier Primary Care Membership, designed to provide our patients with comprehensive, proactive, and accessible healthcare throughout the year. This membership is an excellent option for individuals seeking a consistent, long-term relationship with their primary care provider.

• Standard Monthly Cost: The membership is priced at $150 per month for the full 12-month term.
• Initial Payment Structure: To simplify administration and ensure commitment, the first half of the membership term is paid upfront. A payment of $900 is due upon enrollment, covering the first six (6) months of service.
• Subsequent Payment: The remaining balance for the subsequent six (6) months of the membership is due in the seventh month of the agreement.

Comprehensive Services Included in Your Membership

• Standard Primary Care Services: This includes routine annual physical exams, wellness check-ups, chronic disease management (e.g., hypertension, diabetes), preventative screenings, and general health consultations.
• Same-Day Sick Visits: We understand that illness can strike without warning. Members are guaranteed access during business hours to 2 same-day appointments monthly for acute illnesses and minor injuries, minimizing wait times and the need for urgent care facilities.
• Routine Lab Reviews: Your membership covers the time and expertise required for a thorough review and interpretation of routine blood work and lab results with your provider, ensuring you understand the implications for your health. Please note: the actual cost of the lab work itself is typically billed separately by the lab facility, but the consultation/review is included.

Important Membership Exclusions

• Specialized Services Not Covered: This membership does not cover the cost of appointments, consultations, medication, or supplies related to:
  • Hormone Therapy (e.g., Bio-Identical Hormone Replacement Therapy - BHRT)
  • Medical Weight Loss Programs: Services involving prescription weight loss medications, advanced nutritional counseling, or ongoing specialized monitoring.
• Billing for Excluded Services: If you require or elect to receive these specialized services, all related appointments, medications, and treatments will be billed separately at our standard rates or through a specialized program fee.
